I like the form, but number 5 seems like it would cause some people to take a second look. I might add something like, "Although these conditions are rare we will exert every effort to give the best cleaning possible." I guess if it ain't broke don't fix it, but I can well imagine what would happen if I showed that to a customer before cleaning their furniture. I added the term rare, because in my experience those things are for the most part extremely rare. For example, I've only seen maybe one couch bleed, which fortunately I was able to correct, and one shrinkage since 1987 and I've cleaned a fair bit of upholstery. Been doing it since year one. I'm just throwing this out there in case anyone decides to customize the form and make any changes.
